Good Morning, I have always billed 11043 and 11046 together with 11046 being the add-on code. Medicare denied my most recent stating 11046 cannot be billed separately? Since when? I have looked over all the 2024 changes and don't see anything regarding this. I can't get through to Medicare via phone. Has anyone else run into this issue?
