A patient came in for a plantar wart. Documentation indicates it was "pared", 11055 with 078.12 correct? What sort of documentation should the note include? I have only the location and under assessment, pare wart (done). Does anyone have documentation guidelines for 11055? If the note had indicated it was removed using a currette, would that have made it 17110? Thanks for the input.
