I have a patient who came in for a post-op for a BKA (global) visit with our Vascular surgeon and requested removal of "two non-tender, non draining calluses
" removed. The provider documented only that "plantar surface removed with sharp scissors. Skin surface below is intact. No follow up is needed. " This patient does had DM on his problem list ,however there is nothing documenting that the patient was having any issues. They are under the care of a different podiatrist, so this appears to just be as documented, a request by the physician. What are the requirements for 11056? If I query my provider, I would like to know what questions to ask. Thanks so much.
