If we billed for a biopsy 11100 with dx code 238.2. The pathology reveals a dysplastic nevus with margins involved. The patient is brought back in for re-excision. Do we code as another biopsy 11100 or for an excision (example 11403)? Also, would the second visit be coded with the 238.2 dx or would you use the code indicated on the biopsy?

you'd use the excision code if that's what they do(the re-biopsy that they'll do on that tissue is inculded in the code). if the initial biopsy came back as "plastic nevus with margins involved", that's what you'd be coding from for "this" excision - because that's "WHY" they're doing this excision..
