Wiki 11400 and 11402 denials

BCBS denied the following codes billed from POS dermatologist's office for excision benign lesion 0.5 inches on established pt's back (80 y.o. male):
1) DOS I0/10/2023
ICD-10 L728, L570
CPT 99213 -25
CPT 11402

2) Same pt, provider, POS, excision benign lesion 0.2 inches on different area of back
DOS 10/30/2023
ICD-10 L728
CPT 11400
FYI: Demographics, POS, etc. all accurate/confirmed
TCT ins re denials resulted in "...we don't pay for the codes provided..."
Your thoughts would be appreciated.

10 day global so no issue there however the DOS 10/10 charge should be 11400 as this code is .5cm or less.

sorry code is cm charges are correct look at policy maybe dx code isn't payable per policy
