11750 on same toe, one month later

My doctor performed a matrixectomy on the lateral border of a patient's right great toe. At follow-up visits during the post-op period, all indications were good that healing was going well. One month after the initial procedure, the patient presented with swelling in the area again, and the doctor performed another matrixectomy. Medicare LCD states that regrowth usually takes at least four months; how should I code this? This is well outside the global period for the initial procedure, but I can't help feeling there may be an applicable modifier. Or, maybe I should code for 11765, given that the issue was mostly the excess tissue? Thank you so much for any guidance on this!
