Wiki 14060 not receiving payments

Hi Im having problems billing 14060 with 15630 and 15004 although I have spoken to different insurance reps to help figure out why 14060 never gets paid opposed to the 15630 and 15004 I have still not been able to get a good answer. Someone please help!!
