separate line for each - with modifier

You will need a separate line for each, with modifier -59 on second and subsequent codes.

Just a word of caution ... this is a HUGE area (at least 271 sq cm up to 300 sq cm). Be sure your math is correct re the square centimeters and be prepared for a denial, which you will appeal with medical records.
