15777 - code when billing

I am having issues with the 15777 code when billing with either first or second stage breast reconstruction. Ingenix says that this should be billed with the 50 modifier when billed bilaterally, but some of my insurance companies are saying this code can't be billed with a 50 modifier. Any suggestions/advice on this?
