I am trying to put together some E/M coding guidelines for our family practice physicians, and I have read some information that differs from what I understood.

I understood that the 1995 require reporting any of the 8 components of HPI based on the patient's account of the presenting problem(s), and that the 1997 have this requirement, but also allow for the reporting of the status of 3 chronic conditions in order to reach a extended HPI.

In my research, I have found that other coders/trainers are training physicians that the 3 chronic conditions option is allow for either the 1995 or 1997 guidelines.

Would someone please clarify this for me?
