Under Gastrointestinal, one bullet is "Obtain stool sample for occult blood when indicated". How should my provider document this in order to get the comprehensive exam if the test isn't indicated? This is a shaded area so all bullets must be documented. Obviously, an occult blood test is not necessary for every patient or every visit. My thought is to state "occult blood test not indicated". I would love to hear other opinions. Thanks!
 
You are correct it should be documented and I agree with the statement you stated.It tells an auditor that it was reviewed but not needed.
