Doc repaired 2 recurrent ventral hernias through 2 incisions both with mesh. Is it appropriate to code 49565 & 49568 twice appending modifier 59 to the second?

That's what I would do. I would also send it paper with a copy of the op note along with a short letter requesting reimbursement for the two "separate" surgeries. Good Luck!!
