A patient came in for an IUD insertion after her old one fell out at home. The provider inserts a new IUD. In the notes, he says when he went to cut the strings the IUD actually expulsed itself, so he had to insert another IUD. Can I code for 2 insertions and can I code for 2 IUD devices? Also would I use T83.39XA for dx?

I don't believe you can charge the insurance twice for the device when its a failed insertion. You may want to check with the manufacturer to see if they will replace it.
