Wiki 2 or 3 PFSH for Well Child Checks

Hi,
I am wondering if there is documentation regarding if 2 or 3 PFSH are required for an Established patient for a Well Child Check visit. Thank you for your help.
 
Preventive Visits

The guidelines for Preventive visits are the same regardless of age of patient - Direct from CPT 2013 Professional edition: Periodic comprehensive preventive medicine reevaluation and management of an individual including an age and gender appropriate history, examination, counseling/anticipatory guidance/risk factor reduction interventions, and the ordering of laboratory/diagnostic procedures. (emphasis added by FTB)

So there's no counting bullets the way there is in New/Established patient visit.

From CPT Assistant May 2002, "The comprehensive history obtained as part of the preventive medicine service is not problem-oriented, and does not involve a chief complaint or present illness. It does include a comprehensive or interval past, family, and social history, as well as a comprehensive assessment/history of pertinent risk factors. The comprehensive examination performed as part of the preventive medicine service is multi-system, but the extent of the examination is based on the age and gender of the patient, and the risk factors identified."
