2 places of svc, same day denials

Recently I have had a denial for the infusions billed in the office when the patient was then admitted to observation, also outpatient service. The places of service are different, but I am not sure how I should have submitted.
Also had denial for infusion codes on the same day a patient was admitted to hospice. Are all services on admit day bundled, as a global package?
