2 Questions: Polypectomy causes second LE and Fluoroscopy Billing

Hello,

Question 1) Two days earlier, one of our specialist removed various polyps. Now, another one of our specialist made a return to the GI lab to repeat a LE to controll Hem. at the polypectomy site(s). How would I code this? Medicare is the insurance.

Question 2) Our physician does a duodenal stent placement. He states, "X-rays were taken and this was done under fluoroscopic guidance without radiologist present and all interpretations were performed by myself." What is the correct code for this? We had thought it was 76000/26 but was told that is included in the service?
