My understanding is physicians can choose between using time or mdm, for each encounter.
AMA chart is great. but I thought it was only used for mdm?
Yes, starting 01/01/2021 for office services, E/M selection is based on either MDM or total time by clinician on that day.
The AMA chart is the new MDM guide, because it is slightly different than the current MDM guide. Because I like as few papers/charts as possible, next to each E/M, I will add what the time requirements are in case we need to bill based on time.
 
Is there a guideline for calculating Time? Per new guidelines

Is there an example of a chart note that shows when time is used and applicable cpt assigned?
I took a webinar back in July/August that had same cases (encounter note) for us to assign e/m based on MDM and time.
Sometimes coding MDM was a lesser e/m code than if physician was to code based on time.
Obviously depending on physicians documentation
It would be useful if time and MDM had guidelines with examples

Per AMA time includes,
preparing to see the patient (review test)
obtaining and/or reviewing separately obtained history
performing medically appropriate exam and/or evaluation
counseling and educating the patient/family/caregiver
referring and communicating with other hcp(when not separately reported)
documenting clinical info. in the ehr
independently interpreting results (not separately reported) and communicating results to patient/family/caregiver
care coordination (not separately reported)
