209.57 with 45341

Medicare is denying 45341--sigmoidoscopy with 209.57--benign tumor rectum as Diagnosis inconsistent. Doc says Diagnosis is correct. 45341 is use of u/s to examine the rectum, I don't get why NHIC isn't paying.

Anyone else come across this? Did you use a different procedure code--45300 etc?
