Can someone please advise me as to where I can find the LMRP for New Jersey on this code or a list of the acceptable DX codes for this procedure??
 
I don't think your going to find a list of dx codes for this. For me, large bone grafts are used mostly when the patient has an arthrodesis type of procedure and the bone is mixed into the joints that are being fused.
