Wiki 21235 and 15731 with AARP

Hi I have a patient that had a procedure and the codes billed were 15731, 21235 and 17311. I did get prior authorization for the 15731 and 21235. UHC/AARP requested notes so I sent in the visit note, path report, photo of nasal tip, approve authorization sheet from the insurance for those codes, letter from the doctor explaining medical necessity (she would have lost a good part of her nose because of the skin cancer) and the surgical summary. They only paid for the CPT 17311, stating requested documentation not received. I don't know what else I can send in. Any suggestions on how to appeal this?
