Is it ever appropriate to code both 23460 and 29806 in the same session when one repair is for the posterior and the other for the anterior? Thanks.

So...the surgeon repaired one open and one via scope? Although these codes are bundled, if the above is the case and he has documented his reasons for doing one scope and one open, I would try billing on paper with the op note and a letter requesting payment along with medical necessity criteria. You may or may not get paid for both.
