Does anyone know if there is a list of codes that exists that require you to modify your E&M with a -25. I am afraid that I am over using this modifier and would like to know that I am using it correctly. I know what the -25 stands for and I understand the seperate and distinctness. However, what types of services require the -25 ie; labs, xrays, immunizations, injections or just medical procedures??? This is where I would like some guidance and reference please.

There is a book called Coding with Modifiers published by the AMA. It is excellent as a resource for ALL Modifiers. A must have for the library
