Getting conflicting info from the insurance on this one. We billed 99391-25, 99213, 17250 on this November 2012 visit. Insurance denied saying 25 modifier was needed for 99213, then eventually got another denial saying 99213 should not have a 25 modifier.

I always thought it would just be 25 modifier for the 99391 and bill 99213 without the 25 modifier. Am I correct?
