I can't find anything in CMP regarding the 25 modifier supporting the provider just reviewing medications as over and above, any help would be most appreciated; Provider has only stated "for prevention, continue magnesium oxide 400m po and Nortriptyline 20mg po nightly, I am saying this does not qualify as "over and above" Any assistance would be most appreciated. Thank you

Hello, could you provide a little more information? Is the doctor trying to report the advice to continue medications as an E/M service in addition to a procedure performed that day?
 
The example provided does not show Significant and Separate Identifiable when continuing medications.
