What is the modifier sequences for this case?

I would not use a modifier of any kind as there is no relationship between 99214 and 36415 per the CCI edits and I've never needed a 25 modifier on an E/M when a venipuncture was done the same day. Of course, it doesn't mean the 36415 will be paid since the payer may have a policy to always bundle 36415 with an E/M but that's a reimbursement problem, not a coding problem

At my previous company i used to code E/M with 25 while coding 36415 along. Although there is no conflict between E/M and 36415.
