25 modifiers with new patients

Can we bill an office visit with a 25 modifier for a new patient if there is not a seperately identifiable diagnosis for the visit when a procedure is done on the same day? Does ordering and MRI or X-ray etc., constitute enough to bill the office visit with the 25 modifer?
 
I guess I dont understand the question...... your are billing a new patient office visit, what other CPT are you planning on billing??
