if a patient is seen by an ophthalmologist for an already scheduled minor procedure of the right eye and the physician also does an ophthalmoscopy of right and left eye and documents the problems of the left, either new diagnosis or established, would it be appropriate to attach -25 to the eye visit along with all diagnosis then attach just the diagnosis to the right eye for the minor procedure ??
 
I don't know the guideline for your speciality procedures......... but IF an office visit was done, with a different diagnosis, new or established, it is ok to bill with 25 modifier.
 
Yes they will sometimes have up two 5-6 diagnosis. On this patient she had two diagnosis on the right eye and three on the left.
