What is the rationale for NCCI bundling of 28300 & 28740? What scenarios would be appropriate to bypass this edit with a -59 modifier?

If you look at the foot anatomy that is why.
Depending on the joints for the 28740, there might be a reason to unbundle. Like, if they did a fusion at #1-5 (in the picture). I think it depends on where. I don't have a current AAOS global data book, but the old one I have shows 28300 is included in 28740 too. I think maybe if they get the graft from the osteotomy site it is included.
