My provider billed this below and the 28510 was denied. I know the modifier 57 is for an E&M, so would I replace it with the modifier 51, because this is for Medicare, and I thought for Medicare you don't need the 51? I'm confused! Any help would be greatly appreciated.

Usually you don't perform both open & closed treatment on the same toe (T3-left foot 4th digit). I'm guessing that's the reason, without seeing the operative report.
