Appreciate assistance reviewing this op note. Is this 29827 & 29806-59? Surgeon states both performed, insurance denying 29806-59 (BCBS commercial). New to this, what am I overlooking or what is my error? Attached op note

coded 29827 and 29806-59. Denied for modifier 59. Resubmitted as 29827 & 29806, denied 29806: Procedure code incidental to primary procedure, included in allowance for other procedure.
