if a patient comes in for a dual chamber pacemaker generator change, and during the case, the doctor decides to cap the RV lead, but keeps with the new dual can, do you bill this as a dual chamber gen change (33228) or simply a single chamber (33227).
i was always under the impression that you still billed it by the device?
