During single vessel coronary grafting the physician also repairs postinfarction septal defect "we decided to proceed with entering the right ventricle. Multiple sutures were placed around the septum, brought back through the septum of the right side. These were pledgeted sutured and placed over a pericardial patch. The sutures wer tied and cut. The right atrium was the reapproximated with Teflon patch and a punch arteriotomy was performed in the ascending aorta.

Do we use 33545 or 33681 and DX??
745.4 or 429.71???? Help
