Good day,

Can someone explain how the below procedure codes should be reported? My doc billed:

36226
36225 [50]
36222 [50]

Is this appropriate?
 
36222 -36226

Without a report, the codes appear to be okay except for the modifier 50 appended to code 36225. Bill 36226, 36225-59, and 36222-50.
