What documentation is needed for the cardiologist to bill 76376 with the 93312 for 3D transesophageal ECHO? I am seeing lots of information for anesthesiologists and even radiologists but not for cardiologists. Is it the same? This would not be for facility billing. They are being told that they are missing documentation but are not sure what.
