Does the referring doctor need to request the use of 3D reconstruction codes? I have heard differing information on the subject.

In a non-hospital situation, there should be an order. In the hospital situation, technically the radiologist can order / perform 3D without an order from the referring, but it is recommended that he 1) discuss with the referring physician, and 2) document medical necessity.
