Our Radiologists perform 3D with cerebral angiograms with the correct documentation per the guidelines and yet Medicare consistently denies the 3D charges and I have not been able to locate any documentation for Medicare specifically stating their guidelines for this. Help.......

Hello,

Medicare has a LCD policy L30729. when billing 76377

Requires Two diagnosis... a primary diagnosis for patient's medical condition & secondary diagnosis for body area imaged w/ 3D.
