Pt admission 12/17 until 12/21
31575 scope done at bedside during consult on 12/17 then pt taken to OR on same day for direct laryngoscopy and I&D of vallecular abscess.
Pt seen again on 12/18, 12/29, 12/20.
12/20 was for critical care.
Which of these is billable?
thx

The flexible laryngoscopy and direct laryngoscopy both have zero global days. The I&D has 10 global days. So, if the subsequent days, 12/18, 19 and 20 were all for the same reason that the vallecular abscess was incised and drained, those visits are not billable. The visit on 12/18 can be coded with a 25 modifier. But if the doctor is caring for more than the abscess that he/she drained, and it sounds like he/she may be caring for more, given that the patient needed critical care on the 20th, there is a possibility that one, two or all three of those days could be coded with a 24 modifier, unrelated to global. Make sure your diagnosis is unrelated to the vallecular abscess.
