If the diagnosis stated in the heading of an EGD op note is some sort of GI bleed, melena, etc. (also in the H&P), and EGD with ablation or cautery is done, but the body of the provider's note states "non-bleeding" or "currently non-bleeding" lesion... is this still coded as 43255 due to the GI bleed? Or 43270 due to the "non-bleeding" status? I have not been able to find any official guidance clarifying this.
