Is this a 46611 or 45171 and can you tell me how you came to your answer?

Plan:

1. Anal polyp Proceed with anal polypectomy.

Procedures:
In the office operating room. Its position in the jackknife position with Trendelenburg and the buttocks taped apart. The polyp was prolapsed out at about 1 cm in size 11:00 position in lithotomy. The buttocks was taped apart, and the site was thoroughly cleansed with Betadine swabs. A rectal retractor is placed and local anesthesia was obtained with half percent Xylocaine and epinephrine one 200,000. Polyp was grasped with an Allis clamp and held upwards is removed with scissors. The base cauterized with electrocautery. A rectal pack is placed with Adaptic with Polysporin ointment, 4 x 4's, and 4 x 4's over this to leave this in for 24 hours possible return to clinic one day after packing is out and start sitz baths. 3 times a day and apply Nupercainal ointment to the anal area after the baths given prescription for one tube condition. Patient at end of operation is stable.
