Chest x-ray indicates right lower lobe pneumonia.
Physician does not specify anything further than "pneumonia".

Both ICD-9 indicate "lobar" in description.
Is there another way to distinguish between 485 and 486 if physician doesn't give
any descriptors in his documentation?

485 is bronchopneumonia, organism unspecified
486 is pneumonia, organism unspecified
the difference is the location of the infection, per your radiology report use 486
