Wiki 5 excisions with 5 different diagnoses

I am so unsure about how to put 5 different diagnoses on 5 different procedures done on the same day at the same time in our office.
I seem to only be allowed to put 4 diagnoses per claim, but 6 procedures per claim. So how do I link 1 to each when they won't all fit on one claim?

As of Jan 2012 the electronic 5010 transaction gives you 12 dx codes per claim. You need to speak with your vendor and have this changed. As of Jan 6. 2014 the CMS 1500 paper claim has 12 available dx codes
