Is it appropriate to append mod 59 to a procedure done in dr. office? Mod 25 is appended to E/M visit? Dr. destructed lesion from one area, did bx of another site. -59 modifier is appended to both procedure codes, 11100, 11101
17000.

From your description it appears that there was only one lesion destruction and one lesion biopsy...not sure why you have 11101 listed. Only one code would take the -59 but I don't have my CCI edits available to know which code bundles into which...sorry I can't be of more help...
