Does anyone know if this code includes removal of fallopian tubes or ovaries? I found an article from appc that says yes but it's more than 20 years old. I assume it does?

No, a total/subtotal hysterectomy only refers to the uterus and cervix.
In Codify under the lay description, it states

Tips
Because 59525 is an add–on code, do not add a modifier to this code. Note, however, that from a clinical standpoint, a subtotal or total hysterectomy does not include removal of the fallopian tubes or ovaries. If the provider removes them, report code 58700, Salpingectomy, complete or partial, unilateral or bilateral or 58720, Salpingo–oophorectomy, complete or partial, unilateral or bilateral, in addition as a multiple procedure during the same surgical session.
