Doc did a procedure which involved a 5 cm x 7 cm of application of Integra. Found code which is Q4104-integra bilayer Wound Matrix -per sq cm- Which then maked this to 35sq cm -please correct me if I am wrong , so how would I bill the 35 SQ CM ?,do I bill as

Q4104 -With total invoice bill
or di I have to bill Q4104 35 times?
