5fu with a pump and mues

I am still having problems with 5fu. I have a denial from Medicare re the mues placed on the drug they refered me to the coding and billing guidelines for billing for the drug that goes through the pump. We do not bill for the pump. I do bill 96416 the prolonged infusion that requiring the use of a pump. If the drug is used for the effectective use of a drug using a dme . I tried the break up on the codes with the 59 modifier and they still got rejected. Would appreciate anyones input.
