Both of these codes require a 5th digit, however 823 is Fracture of the tibia and fibula, code 823.8 is UNSPECIFIED part, closed and 823.9 is UNSPECIFIED part, open

Therefore to assign a fifth digit you need to SPECIFY ._0 tibia alone, ._1 fibula alone or ._2 fibula with tibia

If the 823.8 &.9 are unspecified part, how can you specify which 5th digit is used?

unspecified part meaning unspecified part of the tibia, fibula or tibia w/ fibula.. such as upper end, lower end, or shaft (the other choices in the 823 category) but to code this fx you must know if it is tibia or fibula or fib with tib fx. you can look at the radiology report to find this information and you cannot code without it.
