Wiki 62258 and 61781 any help

My provider has billed 62258 (removal of completeCSF shunt system; with replacement) and add on code 61781 (sterotactic computer-assisted procedure; cranial intradural). we are getting denied for the 61781 as not being billed with a paid primary code. this is happening with more than one payor so not sure what the problem is. Can any help??? :confused:
