I am taking over some rejections and we have to medicare patients that are receiving an injection in the occipital nerve location of lidocaine 1% bupivacaine .25% and depo Medrol for chronic migraine. I have sent the invoices as lido and bupi are billed as J3490.
64405 was used but they are saying "that a qualifying service/procedure has not been received" these all went on same claim. I have read everything I can find and the only other code I see is 64615 but im not sure this classifys as chemodenervation...I would appreciate any help that I can get regarding this, or a direction to take.
