The provider note reads: The area over the left lesser occipital nerve was prepped with alcohol times three. A 5/8" gauge needle was used to infiltrate the area around the nerve. This was repeated at the remaining nerves. Would this be coded 64450 x2, once for the Left Lesser nerve target and once for the "remaining nerves"?
 
Hi there, based on that documentation you'd report one unit of service. From the 2023 CPT manual:

Codes 64400-64450, 64454 describe the injection of an anesthetic agent(s) and/or steroid into a nerve plexus, nerve, or branch. These codes are reported once per nerve plexus, nerve, or branch as described in the descriptor regardless of the number of injections performed along the nerve plexus, nerve, or branch described by the code.
