Can 64450 be billed by the surgeon along with a surgical procedure or is it only for anesthesiology?

Typically no. It will bundle in NCCI edits with most procedures, but it's also right there in the CPT book. I had to copy this and show a doc who was constantly trying to bill digital blocks in the office with his procedures.
